Bereavement Grant
The Bereavement Grant is available for those people who have lost an immediate family member in the February 2011 Christchurch earthquake.
An ‘immediate family member’ includes: the deceased persons spouse, de-facto partner, parent, sibling or child.
Where an application is successful, the amount of the Bereavement Grant will be $10,000.
Only one application will be considered in relation to each deceased person.
If an application is being submitted on behalf of a child under 16 years old, please have a second party complete the application form.
Before you apply
Please ensure you have the following:
- A copy of the Death Certificate for the deceased OR
- Other appropriate documentation
Details of the bank account into which you would like the grant paid if this application is successful, that shows your name and bank account number:
- A printed deposit slip for this bank OR
- A copy of a bank statement number
